Dr. Haas is a qualified physician in Louisiana to write recommendations for medical marijuana. He was one of the first ten physicians to qualify. He has been an advocate for medical marijuana for his Hospice patients for many years.
Medical Marijuana in Louisiana
In 2015, the state of Louisiana passed a legislative act for the legalization of medical marijuana. It is anticipated that by Summer 2019, Louisiana will make available access to this treatment option. Unlike normal prescriptions, recommendations for medical marijuana will be needed. These recommendations will be sent to the state-run dispensaries to be fulfilled. Willow pharmacy in Madisonville (St. Tammany Parish) is one such dispensary. More information on the logistics of fulfillment will be coming soon.
Who Qualifies for Medical Marijuana?
You must be a resident of Louisiana. You must have an unexpired government issued photo ID. Examples include: LA driver’s license, LA state ID card, military ID card or U.S. passport. The picture on the ID must be an accurate representation of a person’s current appearance. There are no age restrictions on who can receive treatment. You must have a qualifying medical condition as well as a history of failed conventional treatments.

• Post-Traumatic Stress Disorder

• Autism Spectrum Disorder (*Specialist referral required)
Failed Conventional Treatments
Patients must have a history of failed conventional treatments before medical marijuana can be initiated.

We do NOT accept walk-ins. All consultations must be scheduled at least one week in advance.
A $50.00 non-refundable deposit is collected at the time a new patient consultation is scheduled. The deposit is applied towards your $225.00 consultation fee. You are allowed to reschedule your appointment with a 48-hour notice. If your appointment is missed for any reason, your deposit will be forfeited.
We do not offer consultations, medical advice, medical services, medical care or medical marijuana recommendation refills after hours.
Items to Bring
- Relevant Medical Records. Medical records must be dated within the past 12 months and from your current treating physician. The records must be on paper and state your qualifying medical condition, how long you’ve had your medical condition, your previous treatments and your current treatment plan. Also, please include a list of all medications you are presently on or have tried, including marijuana products. (Can also be faxed to 985-892-8063)
- Unexpired government issued photo Examples include: Louisians driver’s license, LA state ID card, or US passport. The picture of the ID must be an accurate representation of a patient’s current appearance.
- Proof of Louisiana residency dated within the past 6 months. This is only needed if you do not have a valid LA Driver’s License or LA state issued ID. Examples include: a utility bill, bank statement, or government letter (all must contain a Louisiana address).
Payment methods: We accept credit cards and cash. We do not accept checks.
Since the federal government at this time does not approve of medical marijuana, insurance companies most likely will not cover the cost of office visits needed nor the medication.
Certifications / Recertifications $225.00
Medical marijuana patients will be seen in our office every 3-6 months depending on individual needs, medical issues, guidelines, or legislation determined by the State of Louisiana. Dr. Haas will confirm your qualifying medical diagnosis, discuss your symptoms and goals. Review your medical history including medical marijuana usage, formulate an individualized treatment plan, provide follow up plans, discuss any questions you may have, and, if deemed appropriate, submit a medical marijuana recommendation to a licensed dispensary.
THC and CBD are both cannabinoids derived from the cannabis plant. THC is one of the active ingredients in marijuana. This is the ingredient that is a DEA Schedule I federally non-legal drug. CBD is currently not regulated by the Federal government. The combination of THC and CBD gives certain results depending on the genetics of the plant. Recommendations from Dr. Haas will contain total dosage and ratios of THC and CBD. Each condition and symptom being treated has different ratios of these two vital chemicals in the marijuana plant. Adjusting the ratios of these two chemicals for optimal symptom relief is an important function of each in-person visit.
The only clinic service we provide is medical marijuana recommendations for patients participating in the Louisiana medical marijuana program.
You will need to maintain a relationship with other physicians for primary follow-up of your qualifying medical condition.
If we are unable to provide a recommendation because medical marijuana product has not yet been released, or is not available, we will submit a recommendation when product becomes available.
Fees are non- refundable for any reason.
Recommendation refill consultations: $50.00
Because the State of Louisiana limits medical marijuana recommendations to 30 days, refill consultations are usually necessary in-between the Certification / Recertification appointments. For patient convenience, these refill consultations will last approximately 15 minutes.
We do not issue refill recommendations without a doctor’s consultation. We ask that our patients pay attention to his/her remaining doses of medication and schedule a refill appointment at least 1 week in advance of expiration of an existing recommendation.
Fees are non- refundable for any reason.
Patients may incur additional fees for services which are not included in the consultation fees. These services include, but are not limited to, completion of forms, school, work excuses, copying medical records, etc. These fees will be disclosed prior to the service
We cannot guarantee the price of the product, dosage limitations, whether the product will meet your needs, when the product will be available, when your local dispensary will be open, if all eligible patients will be able to obtain product, or any other issues outside of our control.
In addition, we are required to follow all applicable medical guidelines, state laws, and medical board regulations regarding general medical practice, treatment of chronic pain, and recommending of medical marijuana.
These guidelines, laws, and regulations can change suddenly and without notice. Your ability to receive a medical marijuana recommendation could be adversely affected by changes to any of the above.
